Read the QT line before you praise a 10-20 mg vardenafil review

Potency per milligram is a binding boast, not a reason to pick Levitra over a 50 mg sildenafil tablet. Vardenafil's usual start is 10 mg about an hour before, then 20 mg or 5 mg. What the reviews often skip is the QT paragraph. This briefing puts that paragraph first. The molecule sheet is vardenafil. A QT history can close this column while the other two stay discussable. Congenital long QT, or Class IA / Class III antiarrhythmics, is an avoid, not a 'use 5 mg and see'. If you still need the three-way grid, open the PDE5 comparison.

Does a smaller milligram mean a stronger tablet?

Does a smaller milligram mean you got more drug for less money? No. 10 mg vardenafil and 50 mg sildenafil were not titrated on a shared ruler. Bioavailability of vardenafil is about 15%. Peak after a 20 mg fasted dose sits between 30 minutes and 2 hours, median 60. Terminal half-life of parent and M1 is about 4-5 hours. That is a short evening, like sildenafil, not a tadalafil weekend.

M1, the desethyl metabolite, is about 26% of parent in plasma and about 28% as potent on PDE5 in vitro, so it accounts for about 7% of total activity. That is a PK footnote, not a marketing boast. If a review calls Levitra 'the potent one' and never mentions QT or food, the review is selling a milligram myth.

Does food still move a 10 mg peak?

High-fat meals still cut the peak. Two food-effect studies dropped Cmax by 18% to 50%. The tablet may be taken with or without food. That permission is not a promise of the fasted median 60-minute peak. If last time was late and flat, fix the plate before you decide 10 mg is 'not potent enough' and climb to 20 mg.

Sildenafil's meal hit is a mean 29% Cmax cut and a 60-minute Tmax delay. Different study, same practical advice: greasy dinner first is a bad way to judge a short PDE5 tablet. Who should skip Levitra because of the ECG? People with congenital QT prolongation, and people on Class IA or Class III antiarrhythmics. The named examples are quinidine, procainamide, amiodarone and sotalol. That list is not decorative. The study behind the warning used 59 healthy men. Therapeutic 10 mg and supra-therapeutic 80 mg moved QTc in the same band as moxifloxacin 400 mg, the usual positive control.

A later pairing with another QT-active drug added more QTc than either drug alone. Healthy-volunteer milliseconds are not a clinic promise that your interval will stay pretty. If the tracing is unknown and the medicine list includes a rhythm drug, I do not 'trial 10 mg'. I leave the column shut and look at tadalafil or sildenafil if the class is otherwise allowed.

Is there a vardenafil-specific nitrate wait I can count on?

The nitrate wait is not a number you can take to the nightstand. The label says a suitable interval has not been determined. In healthy middle-aged men, vardenafil 20 mg potentiated sublingual nitroglycerin at 1 and 4 hours (blood pressure) and raised heart rate out to 8 hours. Those effects were not seen when the 20 mg was taken 24 hours before the nitroglycerin. That is a healthy-volunteer observation. It is not a protocol for someone with ischemic heart disease.

Nitrates and nitric oxide donors stay contraindicated. Riociguat stays contraindicated. Ritonavir is a different kind of stop: 49-fold AUC, 13-fold Cmax, half-life stretched to 26 hours; max 2.5 mg every 72 hours. Indinavir: do not exceed 2.5 mg in 24 hours. Those are not 'potent tablet' selling points.

What should a generic vardenafil review actually check?

Generic reviews that skip the ECG are shopping notes, not safety notes. Check the milligram (2.5 / 5 / 10 / 20), whether the tablet is the film-coated product (Staxyn 10 mg ODT is not mg-for-mg interchangeable), and whether anyone asked about QT drugs. Brand Levitra film-coated tablets were discontinued in some markets; the generic film-coated tablet is still the labelled object of this page.

Headache, flush, rhinitis and dyspepsia rose with dose in flexible-dose trials that started at 10 mg. That is expected class noise. It is not a QT surrogate. Dizziness after a first 20 mg on tamsulosin is a blood-pressure story, also expected if the stack was sloppy.

Renal cuts are milder here than on tadalafil's daily line. Mild impairment looked like normal controls. Moderate and severe groups ran AUC about 20-30% higher. Dialysis was not studied. I would still rather see the creatinine than assume 20 mg is 'fine because kidneys only matter for Cialis'. The QT avoid list does not get lighter when eGFR is normal.

Sudden vision or hearing loss, chest pain, and an erection past 4 hours are the same emergency sentences as on the other two columns. A vardenafil review that spends 800 words on 'potency' and one line on those stops is not a review I would trust. Write the avoid list in the same paragraph as the milligram, not under a fold.

Why does sixty-five change the first tablet?

Age sixty-five changes the starting conversation because older men ran higher levels: mean Cmax 34% higher, AUC 52% higher than men 18-45. Phase 3 still included hundreds of older patients without a separate efficacy collapse, and the label still says consider 5 mg as the start at 65 and above.

Moderate hepatic impairment (Child-Pugh B) is a harder cut: start 5 mg, do not exceed 10 mg. After 10 mg, those volunteers had Cmax up 130% and AUC up 160%. Severe hepatic impairment was not studied. Stable alpha-blocker therapy: start vardenafil at 5 mg. Unstable alpha-blockade plus a first 20 mg is how people get hypotensive outliers.

Which pills on the nightstand are the Class IA / III problem?

Quinidine, procainamide, amiodarone and sotalol are the names the label bothers to spell. Other QT-prolonging medicines exist. The instruction is to consider them when the history already includes a long QT or a known QT drug, not to run a chemistry seminar at the pharmacy counter.

I will not turn this briefing into a complete QT catalogue. If the cardiologist already said the interval is long, believe that sentence. If the only story is 'he has palpitations', that is an ECG appointment, not a Levitra sample. Hypokalaemia is on the European risk list for QTc-active drugs including vardenafil. Correct the potassium story before anyone debates 10 versus 20 mg.

Is a 10 mg melt tab the same order as a 10 mg film-coated tablet?

Staxyn is a 10 mg orally disintegrating vardenafil tablet. It is not interchangeable milligram-for-milligram with film-coated Levitra or generic film-coated vardenafil. Different bioavailability, different food rules in that product's own label, same class bans. A review that treats '10 mg vardenafil' as one object across both forms is already sloppy.

This briefing locks the film-coated 10-20 mg path. If someone handed you a melt tab because a shop ran out of the coated tablet, that is a different product conversation with the prescriber, not a silent swap. Brand Levitra film-coated stock disappearing from a market does not retire the generic coated tablet, and it does not make the melt tab a generic of the coated one.

Potent CYP3A4 blockade still dominates either form. Ritonavir's 49-fold AUC jump and 26-hour half-life stretch belong in the chart, not in a footnote under 'potent pill'. If ritonavir is on the list, the labelled ceiling is 2.5 mg every 72 hours, which is already outside the 10-20 mg shopping sentence in the title.

The caution that belongs on the script

Write the QT question on the same scrap as the milligram. Congenital long QT or a Class IA / III drug: do not fill this column. Otherwise 10 mg about an hour before, on a plate that is not a high-fat dare, once a day, is the ordinary start. Age, liver and alpha-blockers pull that start to 5 mg.

I am on amiodarone. A forum said 5 mg Levitra is fine if I watch how I feel. Is that reasonable?

No. Amiodarone is a Class III antiarrhythmic. The label says people on Class IA or III drugs should avoid vardenafil. Feeling fine after 5 mg is not a QT assay. The healthy-man study already showed a QTc move at the ordinary 10 mg, in the same band as moxifloxacin 400 mg.

If a PDE5 tablet is otherwise allowed, the discussable columns are tadalafil or sildenafil, with the shared nitrate ban still in force. I would not run a home titration to prove a forum wrong. Ask the person who manages the amiodarone, with the ECG in the chart, not with a leftover sample.

Not on my say-so from a years-old adjective. Borderline can mean many things, and QT-prolonging drugs can have been added since. Get a current ECG and a current list in front of someone who can read both. Until then this column stays closed.

I realise that is an unsatisfying shopping answer. It is still the right one. A 10 mg generic is not a lighter QT drug than branded 10 mg. The caution travels with vardenafil, not with the logo.

It may mean you needed a different plate. High-fat meals cut vardenafil Cmax by 18% to 50% in the food-effect studies. Climbing to 20 mg to outrun dinner is how headache and flush get worse while the timetable stays sloppy.

Repeat 10 mg on a lighter stomach, about an hour before, with stimulation. If that is still short and QT is clear, 20 mg is the labelled ceiling, once a day. If you are 65 or have a moderate liver story, 5-10 mg is the conversation, not 20 mg on night two.

The labelled start on a stable alpha-blocker is 5 mg, not a casual 10 or 20. Both drugs can drop standing blood pressure. Studies that stacked them saw hypotensive outliers, including a symptomatic 80/60 after 5 mg vardenafil with 10 mg terazosin in one cohort.

If the tamsulosin dose is not stable, do not add vardenafil at home. If both are already prescribed, keep the timing conversation with the prescriber rather than experimenting with 'take them six hours apart' from a comment thread.

Shop copy said wait 24 hours after Levitra and a nitrate spray is safe. They cited a study. True?

The healthy-volunteer study found no potentiation when 20 mg vardenafil was taken 24 hours before nitroglycerin. The same label still contraindicates nitrates and says a suitable interval has not been determined, including for people with ischemic heart disease, who were not the population that 'proves' a home wait.

Shop copy that turns that figure into a protocol is misreading a negative finding in healthy men. If he needs a nitrate, he does not need this tablet. Or sildenafil. Or tadalafil. That is the whole class, written in the comparison.

The same copy often forgets riociguat, which is also contraindicated, and forgets that chest pain after any PDE5 tablet is an ambulance problem. A 24-hour wait you read on a product page is not monitoring.
